Your insurance will be billed at $150/session. They will adjust that amount based on their allowable amount for mental health services. You will be responsible for your copay at time of service, and the allowable amount if you have not yet met your deductible. You are expected to be informed as to how your insurance handles mental health services, and are responsible for $80/session if your insurance rejects your claim and/or does not cover mental health.
Clients who choose to pay for their sessions out-of-pocket will be billed $80/session. When filling out the intake paperwork, you will be asked to put credit card information in for billing purposes. You can put your HSA card in as the credit card if you would like to. I do not accept cash, and only accept checks if there are extenuating circumstances. Paying by credit card through my secure billing system is my preferred method of payment. *Please note, the fee for out-of-pocket sessions increases by $5 on January 1st of each year. Starting 1 January 2020, the fee for out-of-pocket sessions will be $80/session.*
LDS Family Services
If your LDS Bishop will be paying for services, you will receive a form at your first session that you will need to return to me after he has signed it. I match LDS Family Services fee schedule and bill $75/session if the bishop is paying for it.
